Please remember to include proper payment when submitting the forms to your Sensei

Requirements for Dan Testing

Must have attended the last four Seminars (a Camp may count as a Seminar)
Must have permission of their Sensei
Must have fighting gloves and mouthpiece
Three (3) copies Dan Registration form, must be typed and signed by student (regardless of age)
Two (2) passport pictures - colur or black & white
Dan registration fee - US CASH
Dan testing fee - US CASH
Seminar fee
Rank record (testing sheet suplied by Sensei)
It is mandatory to take the Seminar before examination
